Vaccination Program

All licensed clinics must notify every employee of the influenza (seasonal) vaccination requirements and the risks and benefits of vaccination.

We are required to provide or arrange for vaccination of all employees who cannot provide proof of current immunization against influenza (seasonal) , at no cost to the employee, unless the employee declines vaccination.

By December 15 annually — 

All CSI employees and contractors will be required to PROVIDE TO THEIR SUPERVISORS one of the following:

1. OBTAIN flu vaccination and provide written proof of vaccination, OR

2. DECLINE the vaccination. Complete and sign declination form.

An employee who declines vaccination must sign a declination statement and certify that he/she has received information about the risks and benefits of vaccinations.

What You Can Do to Stay Healthy

  •  Stay informed.
  •  Influenza is thought to spread mainly person-to-person through coughing or sneezing of infected people.
  •  Take everyday actions to stay healthy.
  •  Cover your nose and mouth with a tissue when you cough or sneeze.
  •  Throw the tissue in the trash after you use it.
  •  Wash your hands often with soap and water, especially after you cough or sneeze. Alcohol-based hands cleaners are also effective.
  •  Avoid touching your eyes, nose or mouth. Germs spread that way.
  •  Stay home if you get sick. CSI recommends that you stay home from work and limit contact with others to keep from infecting them.
  •  Follow public health advice regarding school closures, avoiding crowds and other social distancing measures.
  •  Find healthy ways to deal with stress and anxiety.
